Faith Formation Registration

Registration for the 2023-2024 school year is CLOSED.

Information for the next school year’s registration will be available JUNE 2024

First time registers, please make sure to bring the documents below to the parish office, along with your completed registration form and the $30 registration fee. If you are not a new register, please make sure to bring in your completed registration form along with the $30 registration fee. The parish office is open Monday through Friday 8:30am-3:00pm. (Please note that we only take cash or checks)

  1. Copy of Birth Certificate
  2. Baptism Certificate
  3. First Communion Certificate 
  4. ID of legal guardian completing the registration

Requirements

First year of First Communion– Child must be in 2nd grade or higher.

Second year of First Communion– Child must be in 3rd grade or higher and must have completed 2 years of Religious Education with good attendance.

First year of Confirmation- Child must be in 9th grade or grader.

Second year of Confirmation- Child must be in 10th grade or greater, must complete 2 years of Religious Education with good attendance.
